Lumbar Cage Design Concepts Based on Anterior Interbody Fusion

 


Anterior lumbar interbody fusion (ALIF) is a surgical technique used to treat persistent back pain, radiculopathy, and neurogenic claudication in patients with degenerative lumbar spine disease, low-grade spondylolisthesis, and pseudoarthrosis who have failed to respond to conservative treatment. Both the surgical procedures and the apparatus necessary for ALIF have improved dramatically during the last half-century. 

The ALIF cage and the materials used in it have progressed substantially in recent years, with the shared objective of improving fusion rates and optimising clinical outcomes.

Lumbar Cage – For Recovering Your vertebral Bodies

Lumbar cages were created to allow the spine to recover between the vertebral bodies instead than along the back. A more stable fusion can be achieved by removing the disc between the vertebral bodies and replacing it with cages and bone graft. In the past, this sort of fusion was tried by simply replacing the disc with a bone graft. 

However, this resulted in the graft collapsing and a low healing rate. By using metallic or carbon fibre fusion cages, structural support is supplied from the cage while bone graft or bone substitutes are used to heal both within and outside the cage. Disc degeneration is the most prevalent reason for an anterior fusion with cages.

Risk Factors

The medical record system at our institute looked back at 667 individuals who had lumbar interbody fusion surgery between November 2011 and December 2014. Eight patients with cage retropulsion were included in the study, two of whom had their original surgery in another facility. 



Visual analogue scores (VAS) and the Oswestry Disability Index were used to assess the clinical results (ODI). The incidence of cage retropulsion was studied using plain radiographs and three-dimensional computed tomography images.
